Below, please find a printable version of our enrollment form which may be faxed to us.
Instructions on how to deliver the form are included in the form. Although it says in the form that the enrollment sheet may be mailed to us with a personal check, this is no longer an option due to unreliability of the postal service. If you are not comfortable with enrolling online, you may fax us the printable form at 502-7761-4342, with your Visa card number, name as it appears on the card, billing address, and expiration date. Email us at info@celasmaya.edu.gt if you don't have a Visa card and want to reserve your space.
